This Website Privacy Policy explains how Texas Specialty Physical Therapy, Inc., doing business as PT ReVolution of Texas ("PT ReVolution," "we," "us," or "our"), collects, uses, and protects information when you visit ptrevolution.health, contact us, or sign up for a workshop.
Once you become a patient, the health information in your treatment record is protected by federal and Texas privacy law and handled as described in our Notice of Privacy Practices. If this policy and that notice ever conflict about your health information, the Notice of Privacy Practices controls.
Please don't send health details through general forms
Our general contact, consultation-request, and workshop forms, and ordinary email, are for scheduling and general questions. Please do not include detailed medical history, diagnoses, insurance numbers, or other sensitive health information in them. We will collect that securely at your visit.
Information we collect
Information you give us. When you request a consultation, register for a workshop, join our email or text list, or contact us, we collect what you provide, such as your name, email address, phone number, preferred location, and your message.
Secure online intake. If you complete new patient paperwork online, it is transmitted to a secure, HIPAA-compliant service provider that has signed a business associate agreement with us. It is treated as protected health information under our Notice of Privacy Practices.
Information collected automatically. Like most websites, ours automatically collects certain technical information, such as your IP address, browser and device type, pages viewed, referring pages, and the date and time of your visit. We collect this through cookies and similar technologies described below.

Cookies, analytics, and tracking technologies
We use cookies and similar tools that are necessary for the website to work, and analytics tools that tell us, in aggregate, how the site is used.
We do not place third-party advertising or tracking pixels on our secure intake, patient paperwork, or any page where you enter health information, and we do not use tracking technologies to share health information with advertising platforms.
